Hiking Trip



O & G Posting

O & G Posting in Batu Pahat has been a fruitful period for all of us who come here, We are required to conduct 5 deliveries from 0 experience and 2 episiotomy. Of course, all these can be achieved "fighting" with the student nurses and the community nurses.

The first 2 weeks has been a busy period as all the portfolio (reports x 2) which are long winded and telling grandma story need to be submitted by dateline. Operation Theatre quota also need to be fulfilled as we are required to scrub in for OT with 2 cases of Obstetric cases and 2 cases of gynaecological cases. It will be easier to scrub in for LSCS and Bilateral Tubal Ligation (BTL)as it is counted as 1 obstetric and 1 gynaecological procedure. On top of all that, there is possibility of getting detention class over the weekend if the ward jobs were not covered well.

After the first 2 weeks, everyone become less tense and starting to play slow as almost all of them would have fulfilled their quota. For me, i already signed 5 of my deliveries in the first week and signed another 3 of them for assisting deliveries on the second week. Planning to do more this week for the detention and weekend call. For OT procedure, i also managed to signed all my cases of 4 in the OT week. Somehow life just gets easier on me for this week.

Long Forgotten..

In the past, i used to

1) indulged in nice songs/music sometime even until the next morning without sleeping. it purified my soul and emotion..but i no longer

2) enjoy reading books ,plenty of thick books, fiction stories..found so many of excitement there inside the cover itself..now? hardly finish a chapter...

3) involve myself in martial art, looks so high upon what i might achieve with what my potential will be..until i realised the other way around..

4)work very hard just to prove that i am really on the right track of taking up medicine. Almost anything come up as common sense. but it no longer appear to be the truth

5) so confidence that i can remember even i have just read once on certain topic and be able to recollect whatever necessary, but it seems impossible now..


What has happened to me? i wonder...

Acute Immune Thrombocytopenic Purpura (ITP)

1) ITP :

Immune Thrombocytopenic Purpura (ITP) is believed to be a immunological reaction which follows an acute viral infection causing antibody binding to platelet membrane thereby allowing destruction of the antibody-coated platelets to be destroyed in reticuloendothelial system. It’s usually a clinical diagnosis with exclusion. However, those presenting with atypical features such as knee pain, inguinal mass and back pain should be investigated with bone marrow aspiration to rule out leukemia1. Platelet – associated IgG can also be tested in him and 90% of those with ITP will be tested positive1.

2) Approach in treating acute ITP :

As most of the patients of ITP achieve spontaneous remission within 8-12 weeks, it’s recommended that only those patients having platelet count of less than 20,000/mcL and significant mucous membrane bleeding or platelet count < 10,000/mcL and minor purpura be treated. He therefore fulfilled the criteria. First line of treatment would be using Prenisolone 1-2mg/kg/day until platelet count becomes normal and taper off. Prednisolone alone would be able to achieve 70% remission rate. Otherwise, intravenous gamma globulin is also effective in children usually given over 5 days or 1g/kg/day for 2 days. It’s proved to enable platelet count raise more rapidly and cost-effective than corticosteroid in acute ITP3. Platelet transfusion however is to be reserved for life threatening condition only whereas splenectomy is not indicated in the case of ITP unless severe uncontrollable haemorrhage developed. Research shows that about 22-27% of those with acute ITP will develop chronic ITP that persisted more than 6 months 4. It’s shown that active treatment do accelerate platelet count recovery however do not avert the development of chronic ITP or reduce morbidity during follow up 4. However, treatment with immunoglobulin G , intravenous Anti-D or high-dose pulse steroids have been showing promising results in delaying the need of splenectomy in chronic ITP.

Avoidable..

I had my lovely egg smashed on the floor this morning just because i am not careful enough..all the protein just splashed on the floor ..Wasted

I have started my regime of protein increasing diet by taking eggs white but not sure whether it will be working. Hopefully it will..

Anyway, about the falling egg, its just to remind me not to do something which will lead to bad stuff because i am careless. Worst when it's something which i am expecting

Canthus twitching

It's been few days after that my right eyes external canthus has been twitching involuntarily.

Hmm...Not really signalling something good. I think it's giving me hint that something bad is coming.

Well, i am not sure if it's true enough. On the first day of twitching itself, my external hard disc somehow was claimed by my computer as "corrupted and unreadable"; And that was really of a "great help".

I promised TY to send over some stuff through the internet but finally didn't manage to send it as my external hard disc crashed. At that moment, it was really creating a sense of impending doom as all my data including my academic stuff that i will be needing in sem 10 are all inside and i have just got 1 copy of it

Sigh...i wasted almost 1-2 hours just to find the solution to it..Luckily, i manage to repair it using the repair procedure..Or else, all the data would have been lost into ash..

Today itself, i had my shoe torn with "Open mouth view" to become a flip-flop after V accidentally step on it. Well, i am expecting it to be worn out soon but not so soon...real sad. Guess i will have to send it to the butler shop to have it repaired. I likes that shoe as it was a comforting shoe and the only shoe which lasted me for about 2 years without needing to change one. I am considered lucky as it only got torn off at the end of the day and not the start of the day. Otherwise, i just can't imagine how will be the day like..haha

Aging = Seeing More Weddings & Funeral..

Perhaps, as the days and time keep flying off ; we might not even realise that we are actually getting older and older as the time leave some track on our body.

And, i am reminded today that one of the thing which might make you realise that you are actually getting old is through SEEING MORE WEDDINGS AND FUNERALS.

True enough, as we grow up to a certain age; we might be realising that more and more of our old-time friend or relatives are getting married. Certainly, you are seeing even more funeral rituals going on in the town if you are living in a small town.

How true the statement is...as reminded. I guess i am facing it too..since long time ago when i wasn't really concern about it..

Catastrophic Rain...

Today is another day of my monthly hypnotherapy class. But, in comparison to when i was studying for my certificate and diploma, i was actually taking public transport and "Bus No 11" almost every month when i had the class.

When i was actually reaching the junction before turning into MU entrance, there start the big drops of tear from the sky and in a very short instance; it became a torrential rain which actually can wet the whole of my clothes when if i had to actually just stay in the rain for 5 seconds without cover.

I was thinking back how i was actually so fortunate the last time of being able to walk "safely" until i reached my hypnosis classroom in UM without actually having my self getting soaked.. If i were to talk and public transport for today and then walk inside UM for about 10 minutes to reach the class, i would be certainly fully soaked..haha

One Simple Mistake

i wasn't realise until recently that i have made the biggest mistakes in house number 587. Now, everyone in this house is like seeing ghost when we see each other..Well, maybe i am exaggerating it too much..haha

But, i think it was because of my studying habit which was usually self rather than group study that lead me to this. It was our first posting in surgery when he initially saw the notes i made about neck swelling. Basically, it summarized everything about neck swelling in a paper. He was kind enough to ask if we could start sharing notes and etc but somehow at that moment i was rather giving excuses not to do that.

I don't really know why i did that. perhaps i was more comfortable studying alone. I am not too sure..Anyway, now he no longer share things and he started closing his door physically, i am not sure if it is psychologically..haha

Anyway, hope that things would be better off in the future..

Distractions..

i was supposed to have a peaceful session with my hypnotherapy class today. but a series of calls and messages came and distracted me..

it was about accommodation in BP, cos we are going to stay there for our final semester. However, it didnt turn up well..cos we have 4 person instead of a 3-person house, guess wat? we need to book for 2 houses but don't know who else will be staying with us..sigh...need another 2 housemates.

And, thats the moment where i cant find any way out of the trouble and ended up not knowing what i am studying of today..really a big headache...that was also the moment where i find myself very disturbed by this kind of simple little things..haih..

Staring Eyes..

I knew it on the spot when HL was aiming at V this afternoon. The staring was so powerful and it's a 100% thorough scanning which probably more specialised than any CT-scan or MRI available. He scanned from top to bottom without missing any spot.

I , who was there, feel so embarrass though i wasn't the one who was scanned. Funny..
The same situation also happened last year during chinese new year. The way the BS guy was looking at her (KL), i knew he was totally attracted and will be going for her. And, my suspicion didn't go wrong. The messages that came through my handphone is getting lesser and lesser drastically . They were together only after about 2 months after the incident. Well..who am i to judge that right? haha

H1N1 confirmed in Msia?




Wow...it's kinda freaky to hear that malaysia finally breakings the record and has announced the first case of H1N1 Flu. Worst of all, it's in sungai Buluh hospitals which is rather near to Seremban? And, to complicate all these..all the passangers taking the same flights with the infected need to be traced...

Who knows how many would have been infected , all that we know - H1N1 can infect anyone the day before the flu symptoms arise..Creepy.Imagined all the houseman working in the hospital who might not know what they are facing..

Ultimate Drops, Finally !!!



Finally, we are all glad after hearing the rustling sounds of the strong wind with hustling sounds of the incoming torrential rain..

Here is Seremban, it was really hot like hell that everyday was like being in the dessert for everyone of us. Not a single drop of rain fall during this 1 month period with the excessively high temperature which almost drove us crazy.

The rain continued throughout the night until the next morning where i decided to off the alarm and keep on sleeping until i really need to wake up to go for class.. Haha\

and. the next day, my car is as shiny as it should be..save all my effort of washing the car..so happy..

Radiology Posting




We have started with Radiological Posting(Sem 9) since yesterday,it's a 2 week rotation posting whereby we need to learn about the essential radiological investigations in the hospitals which include Ultrasound (U/L), Plain Radiograph, CT scan, MRI, Fluoroscopy, Intravenous Urogram (IVU)and mammogram.

All these are available for us to access with the exception to mammogram of course. It's for patient's privacy.

Somehow, it has been scheduled that Tue, Thurs and Fri to be Fluoroscopy day which is the investigating machine for Barium Studies and Cystouretrogram.

On today itself, we saw 2 cases of cystorethrogram, 1 case of Barium Meal and Another case of Barium Enema.

Most interestingly, we tasted the Barrium Sulphate which is the contrast used in the study. Well.. it's doesn't taste so bad but we didn't really took so much of course. Orelse, we will surely be running to the toilet. Haha

Barium Meal itself is not a comfortable test as patient need to swallow about 6 small cups of Barium Sulphate so that a few view of radiograph can be taken to see the abnormalities in the oesophagus while standing.

On the other hand, Barium Enema can be a real havoc. It's infused into the patient's rectum via a large catether and later air is also pumped in so that a double contrast barium enema can be demonstrated on the radiograph. Besides that, it requires various turning of position so that the entire colon could be coated to view the entire colonic mucosae in order not to miss any abnormalities.

It's been an interesting day today that we managed to see all the interesting procedures. if we were to stay in MRI or CT scan room, probably there will be lot of fungus growing over there..haha

But, guess what? Anaesthetist claimed the 2nd ranking..unbelievable..

Emotional Pain Can Be As Pain or More...





Read from a medical book a few days ago that emotional pain, the agony experienced by the "heart" couldn't be of any deceiving as compared to the real pain from a lacerating wound with a sharp knife...

How interesting ... though i know that this fact is indeed more truthful than any other thing, it's just how amazing that we can simply denied it when the science is still not able to prove it.

Probably someday will come, there will be something to cure wounded "heart"..haha
Or, maybe it is just up to us to get over with the pain ???

Iron Overload in Thalassaemic Patients



It it a need for patients with thalassaemia to be transfused with blood at least once a month especially when they are of thalassaemia major which is the severe form that need blood tranfusion.

And, sadly to say; there is no medication in this world that can be given without any side effect. Though blood tranfusion helped them to prolonged their life as campared to those in the ancient time, it brought up another problem for them - Iron overload.

Those excess iron as a result of repeated tranfusion couldn't possibly be excreted by our body, hence starting to accumulate in our body in the form of ferritin. When in excess, this will lead to a condition called as Haemosiderosis and Haemochromatosis.

Moreover, this excess iron tend to accumulate in the vital organs such as the heart (heart failure/cardiomyopathy), liver (hepatic cirrhosis), pancreas (diabetes mellitus) and endocrine organs (hypopituitarism and hypogonadism).

Therefore, iron chelation therapy is the choice of options in order for them not to get the side effects from repeated blood transfusion. Usually, iron chelation therapy is only needed after 20 transfusion have been done as each unit of blood tranfusion will give them an excess of 200-250mg of iron.

There are 3 form of iron chelating therapy available :

Desferrioxamine

- subcutaneous injection via an infusion pump (5-7x per week) for 8-12 hours
- indicated for those > 3 years old, ferritin > 1000 ug/l after 10-20 tranfusion
- promote urine excretion
- s.e = skin rashes, yersinia infection (fever, abdominal pain & diarrhea)
- cost - RM 500-1000

Deferiprone

- oral tablet form trice daily (tds)
- usually use in combination with desferrioxamine
- chelate iron from heart also
- s.e = agranulocytosis, joint pain,nausea, diarrhea & elevated liver enzymes

Desferasirox (Exjade)

- once daily dose tablet (OD)
- excrete iron via defaecation
- s.e = nausea/vomiting/abdominal pain, rashes,increased creatinine and liver enzymes


Other options available would include Splenectomy and Bone Marrow transplant

Spleenectomy

- when there is hypersplenism, large spleen or excess haemolysis
- indicated for those > 5 years old, transfusion of more than 200-220mls/kg/year.
- pancytopenia
- s.e = increased susceptibility to bacterial infection (meningococcus/ penumococcus/haemophilus influenza b) - need vaccination 4-6 weeks before surgery
thrombocytosis (use aspirin)

Bone Marrow Transplantation

- ideal choice of treatment (cure rate > 80%)
- allograph transplant from HLA-matched siblings
- s.e = infection, graft rejection and graft versus host disease
- greater chance of success rate if compliant to iron therapy, < 2cm of hepatomegaly and no portal fibrosis

Thalassaemia Awareness Talk in HTJ



Attended a talk organized by Negeri Sembilan Thalassaemia Association in conjunction with a pharmaceutical company for lay person on thalassaemia last Saturday which was rather interesting. An one day whereby they educate lay person about the hereditary disease. The programme lasted from 9am to 2pm and was started with short talk from HTJ Pengarah and Head of Department of Paediatric in HTJ.

Amusingly, it's all communicated in Malay language which i think most of the talkers find difficulty with when they have long communicate in English ever since they joined the medical profession.

There was a talk on the overview of thalassaemia disease which they keep on emphasizing that it's not infectious followed by another talk about the management, working opportunity and issue in pregnancy.

Of course, the most interesting part would be from the pharmaceutical company where they have 2 short video summarising the effect of iron overload and the life of thalassaemic patients after they found out the oral solution instead of the injection. undoubtterly, the cost was not shown, just to terrify the public. haha

The lousiest part? it is a talk by the personnel from government working resources. Basicly, she was just not prepared for this talk. She gave a talk on how to get a job for a normal person without any consideration for thalassemic patients who would require more holidays for their blood tranfusion. Bad day for her, as she actually manage to trigger the rage from some of the patients. How impressve...

Anyway, we were glad to have the free guideline for thalasaemmia which would be very useful for us. Though the breakfast and lunch was provided, it was not really a good meal as there was a lot of people attending the talk. Once you are late in queing there goes the 15 minutes just for that.

Gynaecological Posting

As far as it's concerned me, gynaecological(gynae)posting is really hectic that i hardly can make out some time to read. What am i busy with? Oncall almost every other day from 5 to 11pm. Worst of all, we need to stay in the ward from 5 to 7pm when it doesn't make sense at all as it' s the visiting hours for family members.

What are we supposed to do there? standing there and blocking the way when there is already too much crowd? This is a real crap... When someone mentioned that houseman is also there, i really wanted to just slap that ill-looking face. The houseman need to be there for the patient to take care of them. What are we as a tiny little creature in the ward can provide? Not even information when what we know is actually not much...

Perhaps they have misunderstood the real purpose of being in the ward, which is for us to learn. But learning has to be an initiative for everyone and not covering the ward full of patients for the sake of covering. We are there in the ward to learn about managements of patients and not to learn to cover ward for the sake of fearing some lecturer who would come to the day the next morning asking about the patients.

This week has been a little bit crappy due to some unreasonable people who just wanted to cover themselves up. Anyway, i do hope gynae posting for another 3 weeks to be fun and interesting that nobody else ruin it.

GP Posting




As Family Medicine Posting is one of the essential posting for us in fourth year, general practice is also included in the four week of posting.

It was kinda funny when mentioning about GP posting, i only managed to get a GP to be attached on on the last day of submission of the GP details. i got rejected by 6 GP for attachment due to various reasons which include busy with something and they no longer accepting student for attachment. Sigh...

Fortunately, i finally found my GP on the morning of the last day of submission. it's a clinic near my place in Rasah jaya. I only made a call to request for the attachment as i do not want to get rejected again for the seventh time...

Anyway, i found no regret for my attachement with an Indian Dr in the clinic i mentioned. The Dr is funny, knowledgeable and willing to teach; and that is the most important point here. We would discuss about the case when he is seeing patients and we would chat from ground to sky when there is no patient around.

It's really a great pleasure being in his clinic as i learned a lot about being a Dr, he also taught me some life value which i might treasure being a houseman later on; not very later actually.

His ways of handling the patients really make all the patients feel comfortable. Patients are able to trust him and allow him to actually make the best decision for them. That's is the kind of value would anyone treasure the most being a Dr.

Pak Lang Chicken Rice in PD




Hmm... See, i knew i am cursed by someone to be choked on chicken bone but who? It's chicken rice again!!!

This chicken rice shop as entitled as "Pak lang Chicken Rice Shop" is located in the town of Port Dickson slightly north to Port Dickson Hospital.




It's famous for the French Toast and also the chicken Rice either the plain or the special type.

Taste : 8/10 (soup heavily spiced)
Size : Moderate
Price (Special) : Rm 6.00
(Plain) : Rm 4.50
French Toast :
Sanitation : Average
Awaiting Time : 20-30 min
Service : Average
Overall Marking : 8/10

Personally, i would only give 4/10 for the french Toast. Totally not sure
why it is famous.. Maybe the cook lost the standard today. Haha

Ampangan Chicken Rice



Hmm...weird. looks like it's been chicken rice for this whole month... What a "chicken month". Wonder what is wrong with me..haha

Anyway, this is also another recommended chicken rice near Ampangan Clinic (KKA), a left turning right after KKA.

Claimed to be the " World Champion"- maybe in this small world called Ampangan and was up on screen on TV3-local Majalah Tiga.



It has got :

1 plate of corn flavoured rice
1 plate of chicken meat - small to big pieces (depend on your luck)
1 bowl of soup - small and heavily seasoned
1 small plate of chilli

The chicken meat has a nice texture except for the burnt part outside,Rice smells nice and sweet in mouth. The overall size of the meal is moderate to large with 7/10 taste.Sanitation is also not bad. Awaiting time - 15 to 30 minutes

But, service is lacking.
We ordered 5 drinks but 6 was sent to us and no "taugeh"(bean sprouts) was served. What a disappointment!!!

Malay Chicken Rice in Tapah




For those who always come back from the north-south highway going South, it's always worth trying the economy chicken rice at the Tapah Perhentian. It cost RM 4.50 which is actually more expensive compared to last time. Not to be considered economy anymore, Sigh...

Location ?? not as the same row as the front food stall. it's located at the back near the fruit stalls and a souvenir shop.



Anyway, the "cencaluk" and the chilli sauce are nice. No way you can get it anywhere else..haha

Prevention from Osteoporosis with Calcium supplement?

This fact might be true for those whose age still haven exceed 35 years old as the peak bone mass is achieved around that age. Thereby, drinking more milk and taking calcium supplement would likely to increase the bone mass and prevent drastic deterioration of bone mass at old age. In addition, Vitamin D supplement would also helps in Calcium absorption. For those elderly who take calcium supplement, however, in contradict; serve as a poison leading to hypercalcemia. They will have plenty of symptoms including tetanus, convulsion, moan and groan. Advertisements in milk or calcium supplement at old age are actually misleading to the public.

Seremban 3 Chicken Rice



Shamefully, this is the first time i heard about the nice chicken rice in seremban 3 which is near my place, Rasah Jaya.

It's located at the right turning just before reaching the left turning to new Family Store in Rasah Jaya.The other way is on the way to port Dickson, turn left just after the overcross road above.

See the rice? Golden yellowish rice...nice to see and nice to eat..




The chicken is also not bad, slightly poor in texture, but still considered fine.



The Chilli sauce? Sweet and slightly spicy.Fit with the whole chicken rice but i would prefer if it's more spicy.

Locked Outside Again




Is this the first time I am locked outside? No it is not!!!! I have been locked outside of my own house in seremban for countless time due to some good/bad habit of my housemate.

It was already a bad day for me today and there he goes and do something like this. It’s really pisting me off!!!

First of all, my car alarm remote control was giving up on me when I was almost leaving for PD clinic for orthopaedic posting. So, I have to force myself in and drive the car all the way to the workshop even with the car alarm sounding… It’s as if my car is the police car with siren who need to chase the thief.Sigh… What making me more angry, the remote function again as soon as I reached the workshop. Geram!!!!
When in PD, somehow I was really in a bad mood and condition. So, was keeping my mind rather blank. So, there it was my lecturer who was making fun on every one of us. He just likes that….

After everything settled, I still have to go to the car workshop to sort out the remote control problem. I was told there is a shop which can replace the battery for me. So, I have to make my way there finding actually I don’t really know where it’s mentioned that it’s near to family store. So, there I was, going back all the way to the car workshop to ask again..Then, only I know it’s near the old family store. So I drive all the way there to get the battery replaced. Luckily, it’s the remote control which runs out of battery. If it’s my car problem then I would be dead on the spot with slashed from the car workshop.

Then, when i reached home. after unlocking the first and second lock of the gate. There i reach in front of the glass door. I just felt something wrong about the door position. I extend my hand to pull the door apart and to realise it's locked from inside. DAMN IT!!!!!!!!!!!!!!!!
I am really wanting to kill someone now!!! What the hell is all this about? don't he realise that the door was never lock and that door if locked cant be opened from outside? I called the other housemate to oepn the door for me..As i know if i called the one who locked the door, he would be a burned pig in front of me..haha
