Last Night, i met a driver who is absolutely unbelievable. It was early in the night around 8am when i was driving to Hospital in order to clerk a patient presentation tomorrow.
It happened just somewhere near to the traffic light. There was this car with plate number 6677 which came out of the sudden from the side of the road and forced me to go the right lane from the middle lane. I am still okey with that actually, until the moment where i give the traffic signal to go back to the middle lane (left) so that i can turn to the right to the hospital. And, there he was, stupidly accelerating so that he could pass in front of me from the Left where he almost crashed onto my car as there was another car on the left lane. Anyway, me as expected quickly moved slightly to the right and gave off a hon to that "Reckless driver".
Of course, i am not stingy enough to give him a show of hand ....inside the car.haha
Oops...Don't misunderstood. i mean the whole hand and not those you imagined.
And,then........ The traffic light showed Red. So, we all have to stop. He opened the car door and walk out to also give me a show of hand plus some mumble jumble. Sorry, i couldn't hear what he was talking about because the car window was close. By the way, it was not fun at all...Scared the hell out of me...Who would ever know what that kind of driver would do to me or to my car.
Anyway, i have told myself, the next time this kind of thing happen; i am going to be ready with the "Car Alarm" in hand, just in case he does walk over "to say hello". haha
Sigh...I am quite disappointed with some of the drivers i saw in Seremban. I wonder when can they learn how to respect others and also themselves.

Expensive Train Ticket from Singapore!!
In view of the problems which might be arising when i need to come back from Singapore later on, i managed to get around the town for ticket coming back from Singapore. There is no bus ticket to reach Seremban but Kl , therefore i have to resort to train again. Sigh..I wouldn't have the chance to sight-seeing along the way back from Singapore..
And, guess what. I don't know which Idiot in the internet who said that buying the ticket from Malaysia will actually be counted in Ringgit..When i was at the counter buying for the train ticket, I was told RM 60+ as it's actually SGD 26 and not RM 26...Sigh...I am spending more and more for my medical elective trip to Singapore. I just hope that everything will go fine and be more worthwhile than i think...
Anyway, i am still looking forward for this trip there...It's still counted as my first trip "Overseas". Haha..

Screw and Cervical Collar in the Cervical X-Ray???
Yesterday Morning, We were shown a few radiograph by the ENT specialist. There was this particular radiograph/X-ray which we were shown and asked to comment on it. It was an X-ray with minor opacities over the tooth, 2 small opacities on the anterior and posterior atlas with typical linear like pattern on the lower neck.
The tooth part? O f course we all know that it's actually a dental filling. However, no one could really answer on the opaques linear thing on the lower neck until the very end moment where someone suddenly shouted "" Cervical collar" !! and the two opacities? Someone answered screw but no one could actually answer what screw is that used in the orthopaedic.
Well, i think some of us actually noticed what it was until we were convinced that it's maybe a cervical collar. What a funny laugh....
In the end?
Neck -----> Necklace
Atlas there? ----> earrings
What a tricky thing to show us....
And the lessons?
To all radiologist. please remove patient's jewellery before any radiograph is to be taken orelse it will be mistaken by the medical student as "screws" and cervical collar!!!!!

Rinne, Weber and Absolute Bone Conduction Test
1) Rinne Test -->
After vibrating it, it's put in front of the patient's ears to checked if patient can actually hear the ringing sound. Best if you hear it by yourself first to make sure its a good vibration.
Then put it behind the patient ear just above the tip of the mastoid process. Asked the patient if he/ she is hearing the sounds(not feeling the vibration)and instruct sher to inform once she can't hear. Then, the tuning fork should be shifted in front of the patient ear to check if she still hear anything.Test the other side of the ear as well. Positive Rinne test will mean air conduction which is usually better than bone conduction.
2) Weber Test-->
Again,vibrate the tuning fork using the same way that you're comfortable with before it's put on the center of the patient's forehead/chin(better on the forehead). Ask the patient whether he can hear the sound before you proceed asking whether he can appreciate whether both side of the ears hear equally or 1 side of the ear is hearing better.
If the test Lateralise, or patient hears better on one side of the ear, it means he has either sensorineural hearing loss on the opposite ear or he has conductive deafness of the same side. This should however be confirmed with Absolute Bone Conduction test (ABC)
3) Absolute Bone Conduction test -->
This test is done in the assumption that the examiner actually having both normal conductive and seusorineural hearing ability.
The vibrating tuning fork is placed on the mastoid process again with the tragus of the ear pressed to occlude the external ear meatus (obstruct air conduction).The patient will hear if he has a normal sensorineural conduction of hearing.

Gosh!!! I am having Left sided sensorineural hearing loss???
I was in the clinic with a ENT specialist and was requested to examine the patient hearing ability. Basically, i have to show how i carried out Rinne Test, Weber Test and Absolute Bone Conduction Test using 512 Hz tuning fork.
Tuning fork was tuned either by using the index finger and thumb or by hitting it on the tip of elbow/elecranon.
1) Rinne Test -->
After vibrating it, it's put in front of the patient's ears to checked if patient can actually hear the ringing sound. Best if you hear it by yourself first to make sure its a good vibration.

Ask the patient whether he can hear the sound before you proceed asking whether he can appreciate whether both side of the ears hear equally or 1 side of the ear is hearing better.

This is the part i screwed up where the patient can hear with the absolute bone conduction test and i couldn't on the left side. Goshh...so? i am having some sensorineural hearing loss? The ENT specialist tested me again and it's sound like my ear is having some problem. I was send for Pure Tone Audiometri and the test comes out to be normal surprisingly enough...Hmm...so, what's wrong with me? I myself just can't figure it out...

Another Try for Judo
I am starting to get "itchy", i mean in the sense of getting myself on the track to fight again. It has been sometime where i didn't even touch Judo at all. Or should i say, it's had been the past one whole year. i don't seem to getting all my motivation back and was keep thinking about all the hardship of doing Judo.
Well, it supposed to be something fun but probably i didn't realise all the hardwork that i need to pay? Or that, it's actually something that i haven't learn? To enjoy myself in everything? I lost my motivation honestly.
Recalling on my childhood time, i always get my friend to come to my house for a fight. I mean those children fight with bear hand ; not those using all the dangerous weapons. Sometime, i even fight against a few of them at one time. It's always the childhood memory that bring us most happiness. I wonder why?? Haha
It's also this interest that lead me to Karate in my Secondary School in Perak. I was searching for Judo though but ended up nowhere. There was Taekwondo in my Lower secondary school but it was way to expensive at that time for me. So, i didn't manage to join it. Anyway, i was only starting my karate training in the late form 4 when i got motorbike to travel between kedah and Perak by 20 minutes ride. It's after almost 2 years of training that i managed to get my Brown 4 belt in Budokan Karate(武道馆). However, i didn't manage to complete it until Black belt as i was leaving for Local matriculation for further study.
When i was in my Uni time, the search continued, but again i was not able to get any info about it at the beginning. So, i ended up with aikido for the first few months. It was fun at first but still i just feel that it's lacking of something; not really the thing that i wanted it to be. So, i decided to left and try my luck again with Judo.
For this time at the moment, i am having multiple ache now as i am way to long of inactive sedentary lifestyle.

Update for the car issue
It's happening again...My car couldn't start again after my dad go and buzz with it. Haha
This time it was in my hometown at my house. So, i called the mechanic which we're used to find when something wrong with the car. Unfortunately, he was not available for that day.
Somehow, my younger brother managed to get his friend who are more familiar with cars to give a knock knock. Surprisingly enough, it managed to started. More surprisingly, he said that was the first time he doing it. Luckily, my car didn't spoil because of him.haha
So, i spend 3 hours and Rm 70 in the end after driving it to the mechanic for it to be fully dismantled. What a waste of time and money when i can actually get a new 1 for RM 100 over.
